FORMS
A student must have the following forms (filled out completely and signed) on file with the athletic office before clearance is given to participate. ALL FORMS ARE DUE PRIOR TO THE FIRST PRACTICE.
TAPPS Physical Evaluation - Medical History Form (1st page) and Physical Examination Form (2nd page)
The Medical History form must be completed by a parent annually. The Physical Examination form must be completed by a doctor for all new athletes, 7th grade athletes, and returning 9th and 11th grade athletes. Cheerleaders must have annual physicals turned in prior to summer camp.
TAPPS Acknowledgement of Rules Form (2 pages) for 9th- 12th grades
TAPPS Parent and Student Notification Steroid Use Agreement Form

ALL ATHLETIC FEES ARE DUE PRIOR TO THE FIRST PRACTICE.
Football: $175
Volleyball: $100
Basketball: $115
Baseball: $100
Softball: $100
Track: $75
Golf: $100
There is no family maximum. Fees are per athlete.
